The buddy list will cause every member on it to appear on your personal list of people online, and also receive a special designation (+) in the overall user list. Users on your ignore list will have their PMs made invisible and possibly you will not be informed of their posts in your user cp. I use both very little, but they merely require you to type in the name of the user in question.
Public user groups are simply different classifications of members. Currently the only ones in any real use are Member, Banned, Moderator and Administrator. You cannot get one at the moment.
