I agree with your usage 100% (except for the capitalised 'O'). That's the only way I was taught.

Yeah, the second one is right. That's how I always do it, and that's how E.B. White's (et. al) book on writing papers and proper grammar does it, too. XD

Yeah, many times.

My current Finnish teacher doesn't know how to pronounce French names.. I mean she pronounces them wrong. But I don't have the energy to correct her every time..

When I was in ground school, I had a new music teacher who was very young. She made some accidents when she was teaching us to read notes. Me and my friend told her she was wrong but it still took time for her to admit it..

But usually I don't correct teacher when he/she is wrong. Especially if it's an old teacher. He/she will just start to thank me a dozen times and it will make the situation a bit uneasy..
