Echoing some sentiments and adding a few of my own:
-The ORIGINAL Ninja Gaiden series (NES and SNES). I've barely played the newer games, but I know how insane the first ones were.
-Ghost and Goblins. Any of them. Of course, I attribute must of the difficult to those horrid jumping mechanics (I love you, Capcom, but seriously? Even the PSP version has it?)
-Battletoads. I'm not saying anything.
-Godhand. Completely underrated but fantastic game. Insanely hard, as well. Even with numerous upgrades and setting together all those "perfect" little combination attacks before facing a stage, I've managed, on some stages, to die in about ten seconds. With full life.
-Ikaruga. Another fantastic game, but just... AGGGGH.
-Megaman. I find some of the comments about this one kind of funny. Thanks to a wonderful Christmas, I now own all of the original six Megaman NES cartridges (except Megman 3 :C ).
Yes, these games are insanely hard, but after beating each one a couple of times, I've come to notice (for me, anyways) that they seem to get progressively easier. Especially 6; that game seems to just hand out Energy Tanks after awhile (I've repeatedly gotten to the final boss with between 7 to 9 tanks).
Of course, when it comes to Megaman, I think I'm just kind of a freak, so my opinion may not totally count. o-O
And Megaman 9 is still insanely hard.
Oh, and the Battle Network games. Even with the right chips those games can still be [an entirely different kind of] crazy hard.

Oh, and something that tends to bother me (which I might get flamed for): I'm not always sure what to really say about difficulty in many RPGs. Many of the newer ones, granted, do tend to give you better ways of formulating strategies inside of combat, but so many of the older RPGs I don't see so much as "hard" as I do "you just haven't leveled and equipped yourself enough."
It's one of those times where there starts to be a weird line between using a lot of "skill/strategy" compared to just having enough time to finish grinding (and I say this being one of those people that used to grind quite a bit :[ ).
